The Health Facility Survey (HFS) of Family Planning Services

The Health Facility Survey was commissioned by World Vision India in 4 districts of Uttar Pradesh where it implements its Integrated Birth Spacing Project, to assess the knowledge and interest of Family Planning in the community and the quality of FP services provided by the Health Facilities existing in the study districts. It covered 39 health facilities SHC, PHC and District hospitals spread across the 4 districts including the clients and the service providers.


There were twenty five critical indicators identified for the assessment and the study was designed to assess the indicators by using structured questionnaires among the clients and the service providers.

Three set of HFS tools were used to conduct HFS:

  • Facility Audit with selected questions to the health facility manager
  • Observation of client provider interactions on selected clinical procedures
  • Exit interviews with clients departing from the facility


The overall objectives of the Health Facility Survey was to assess existing situation of the quality of family planning services in the project districts and the specific objective was to compare the results on selected variables obtained from two instruments exit interview and client-provider observation; and to assess the existing situation of quality FP services including the client satisfactions and management system (trained human resources on FP, availability of services, equipments, logistic supply etc.) through health facility audit.


New Concept undertook complete survey from development of tools, translation, training, field investigation, supervision, monitoring, data management, analysis to the final report, which was submitted to World Vision India.
